Who can apply for assistance from the Teva Cares Foundation?

To apply, you must currently reside in the United States and be receiving treatment or have a valid prescription from a licensed healthcare provider practicing in the United States.

Is there a fee to apply for assistance from the Teva Cares Foundation?

There is no fee to apply for assistance through Teva Cares. Medicines distributed through Teva Cares are free of charge to eligible patients. Teva Cares is not associated with any organization that may charge a fee to assist with completing enrolment forms for the Program.

If you have questions about the Teva Cares Foundation Patient Assistance Program and would like to speak with a program representative, please call 877-237-4881. We are staffed to assist you Monday through Friday, from 9:00 AM. – 8:00 PM. Eastern Time.

Application Step by Step

You and your physician must complete the application. Your physician may initiate the process, or you may apply online by clicking here or download an application by clicking here or call 1-877-237-4881 to request an application.

How long will it take to find out if my application is approved?

Applications are reviewed in the order they are received. Determination of eligibility is dependent on receiving a complete application and all required documentation. The Program will contact you within 10 business days with a determination of eligibility or to request additional information if needed to complete review of your application.

If I am approved, how will I receive my medicines?

Medicine is shipped free of charge by the Teva Cares program pharmacy. Depending on how the medicine is administered, it will either be delivered to your home or your physician’s office.

How long am I eligible for assistance?

Once approved, you will receive medicine at no cost for up to 12 months. In some cases, medicine is provided through the end of the current calendar year.

How do I re-enroll for assistance?

You must complete a new application every year. Teva Cares will notify you and your physician prior to the end of your eligibility period with instructions on how to re-apply for assistance. 

Can the Teva Cares Foundation help me access a product coupon or help with my co-pay or other out-of-pocket costs?
